FDA Draft Guidance Would Streamline Approvals of Individualized Gene-Editing Therapies

Oct 27, 2026 —

The FDA has released draft guidance that would make it easier to approve individualized gene-editing therapies for rare genetic diseases, a change long sought by rare-disease advocates. The guidance, titled Individualized Human Genome Editing Products, was announced alongside a CMS plan to expand access.

Its central idea is an Individualized Feature Space (IFS): a single biologics license application (BLA) can define pre-set boundaries describing how a genome-editing product may vary from patient to patient. A new patient-specific variant that falls within those boundaries can be added through a streamlined review, relying on the original trials plus laboratory data for the new variant, rather than a new clinical trial each time. The guidance also covers manufacturing, safety testing for unintended edits, and naming.

On the access side, CMS plans to broaden its Cell and Gene Therapy Access Model beyond sickle cell disease, negotiating outcomes-based agreements with manufacturers on behalf of state Medicaid programs, with a request for applications expected in the coming months. Comments on the draft are due within 30 days of Federal Register publication.
